Admissions

Admission Requirements for Patients

New puppies or kittens presented for initial physical exam and immunizations, illness or injury should be accompanied by available pertinent health records, if any, obtained from the previous owner, veterinarian or breeder.

New adult patients presented for routine yearly physical exam and immunizations, illness or injury should be accompanied by any previous medical history that is available.

Pocket pets should be accompanied by any records obtained from the previous owner, veterinarian or pet store where purchased.

Admission Requirements for Boarding and Grooming

All animals must be current on vaccinations recommended by University Parkway Animal Hospital.  Vaccinations must have been administered by a licensed veterinarian.  Written verification by previous hospital records or invoice for services is required.  The following vaccinations are required:

Dogs-Distemper, Adenovirus/hepatitis, Parainfluenza, Parvovirus, Leptospirosis, Bordetella and Rabies.

Cats-Feline distemper (Rhinotracheitis, Calicivirus, Panleukopenia), Chlamydia, Feline Leukemia virus and Rabies.

Ferrets-Distemper and Rabies.

If your pet has not been vaccinated within the past twelve months, it is highly recommended that they receive immunizations at least two weeks prior to boarding.

All animals are expected to be free of fleas, ticks and other external parasites.  If external parasites are found to be present while your pet is boarded or groomed here at the hospital, they will be treated appropriately at the hospital's discretion to remove these specific parasites.  If animals are found to be harboring intestinal parasites during their stay, they will also be treated accordingly for their benefit, and the health and safety of all boarders.  Fees for these services will be charged to the client upon discharge of the animal(s).

Boarders are discharged from the hospital during regular business hours only.
